ReferralsWe will need to arrange to see the person receiving care or support or their advocate in advance of any service commencing. This will enable us to ascertain level of need and if we can provide a safe effective service that will ensure you reach expected outcomes. Once we have agreed a service details of staff, times, frequency, tasks and outcomes will be put into place and a start date will be set. Full details of the service will be made available on service commencement.
Each person receiving care or support will participate in the development or a person centred care or support plan. This plan will detail all aspects of care and support taking into account your short, medium and long term outcomes and goals. The care and support plan will be reviewed on a regular basis to ensure stays current and up to date. Where a local authority or NHS are the commissioners, we will follow agreed protocols and procedures in place. Normally service will commence as quickly as possible. Emergency ReferralsIf we receive an emergency referral this may be handled by our out of hours service. If our emergency rapid response service is required please contact us on our out of hours numbers.
